The difference
Acini di pepe vs Stelline
Both are minuscule, but acini di pepe are round beads the size of a peppercorn, while stelline are tiny flat stars with a center hole. The bead is a solid sphere, the star a thin pressed shape with points. Acini di pepe nearly disappears into broth, stelline keeps its outline.
